Today, I want to talk HRT by way of something that most providers completely overlook: endotoxin.
This concept is front and center in the HRT University Master Course and will play an even bigger role in the Advanced Female HRT Course.
When I first heard about endotoxin as a nurse practitioner student, I thought it sounded like the most “woo-woo” concept I had ever come across. Then I started diving into the work of Dr. Raymond Peat, and suddenly everything began to make sense.
Endotoxin is not fringe science. It is discussed in the literature constantly, yet rarely addressed in clinical practice.
What Is Endotoxin?
Endotoxin, also known as lipopolysaccharide (LPS), is produced by gram-negative bacteria in the gut.
It is one of the most toxic compounds naturally found in the body and triggers a powerful immune response.
In high concentrations, it can cause sepsis.
In lower, chronic amounts, it drives inflammation and long-term disease.
Chronic endotoxin exposure has been linked to:
- Asthma
- Inflammatory bowel disease (IBD)
- Atherosclerosis
- Rheumatoid arthritis
- And yes, polycystic ovarian syndrome (PCOS)
PCOS and the Gut Connection
Most providers were taught that PCOS is primarily a disease of insulin resistance. That idea is not entirely wrong, but it misses the real root cause.
PCOS often begins much earlier with:
- Chronic stress
- Dysregulated cortisol
- Low anti-stress hormones such as progesterone and thyroid
- Breakdown of the gut barrier
This combination allows endotoxin to leak into circulation, ignite inflammation, and alter metabolic signaling.
Over time, this cascade leads to insulin resistance, weight gain, and the hormonal chaos that eventually becomes PCOS.
What the Research Shows
Dr. Peat was discussing this decades ago, and now modern research is validating it.
A 2025 study by Senthilkumar and Arumugam confirmed that the gut microbiota has a strong correlation with PCOS.
Different bacterial species are associated with radically different outcomes in women with gut dysbiosis.
Researchers are even investigating fecal microbiota transplants (FMTs) as a potential therapy for these women. The gut’s impact on female hormone balance cannot be overstated.
Clinical Perspective for HRT Providers
For providers specializing in hormone optimization, this information changes the entire approach to PCOS. It is not just about insulin. It is about the gut-hormone axis.
And one of the most powerful modulators of that axis is progesterone.
Progesterone binds to endotoxin, regulates immune activity, and protects the gut lining.
