PCOS Isn’t Caused by Insulin Resistance

Knowing the true root cause of PCOS is, in my opinion, one of the most important keys to actually fixing it.

If I asked, “What causes PCOS?” most people would immediately say insulin resistance.

That answer is partly true. It is definitely one of the biggest drivers.

But what if I told you it is not the root cause?

What if PCOS is not just a hormonal disorder or an insulin issue?

What if it is actually a metabolic disorder at its core?

Because that is exactly what both the data and physiology suggest.


The Real Beginning of PCOS

Hyperinsulinemia and hyperandrogenism do not just appear out of nowhere.

The true root cause of PCOS is energy failure.

When the mitochondria stop making energy efficiently, everything else begins to unravel — hormones, thyroid, insulin, cortisol, all of it.

This is where Dr. Ray Peat’s work shines. He taught that PCOS is the result of a cascade of energy failures that ripple through the hormonal system.


How It Starts

It starts with chronic stressors, sometimes even in utero.

That early stress elevates cortisol, which suppresses progesterone and thyroid function and initiates the rise of androgens.

As thyroid function slows, metabolism declines. The body produces less heat and energy and more inflammation.

That is when insulin resistance begins to show up — not as the cause, but as a consequence of metabolic slowdown.


The Cascade That Follows

Once metabolism drops, the ovaries begin to overproduce androgens like testosterone. Estrogen starts to dominate, and progesterone falls dramatically.

The result is the familiar picture of PCOS:

  • Cysts on the ovaries
  • Irregular cycles
  • Acne and hair changes
  • Anxiety and mood swings
  • Infertility and fatigue

It all comes back to energy.
