If I had to start over today and open a brand new cash-based clinic, I would begin by building one thing before anything else:
An ecosystem.
Not a website. Not paid ads. Not even a location.
What Is a Clinic Ecosystem?
A clinic ecosystem is a physical or digital space where current and potential patients can engage with you, learn from you, and build trust with you.
It is where they hang out, learn, ask questions, and get to know your voice.
And the best part? You can start building one today—for free.
Why It Matters More Than Marketing
Most new clinic owners focus on the usual suspects:
- Website
- Instagram
- Google ads
- Funnels
But none of those create community. None of them offer repetition, which is what builds trust. Most patients need around 7 cumulative hours of interaction before they feel confident committing to you.
So how do you create that?
Start With a Niche Facebook Group
Even though I am not a huge social media guy, I will be the first to say that a well-niched Facebook group is one of the best ecosystem tools you can build.
Let’s say you are an HRT provider in Florida who loves working with women in their 50s struggling with menopause.
You could create a group called:
“Florida Perimenopause and Menopause Support Group”
It is location-based, specific, and emotionally relevant. That is how you build a searchable community of the right people who are most likely to benefit from your services.
One of my HRTU students, Allison, did this—and it was a game-changer for her practice.
What Content Should You Share in Your Ecosystem?
As your community grows, ask yourself: how can I provide real value?
Here are ideas you can layer into your ecosystem over time:
- A weekly or monthly newsletter (start with 1x/month)
- A podcast with short, honest, high-value rants
- A mini guide or short book for navigating perimenopause
- A monthly Q&A livestream
- A casual, local meetup (even 3 people at Panera is a win)
All of these position you as a Key Person of Influence in your space.
And here is the secret: when people view you as their guide, they no longer compare prices.
They want you.
Simplicity Wins—Every Time
Do not overthink this.
You do not need the perfect logo or lighting setup. Just show up consistently with sincerity and clarity. That alone will put you ahead of 95 percent of other providers.
Once your ecosystem is alive, your business will grow as a natural side effect of you being of value to others.
