Sperm Counts Are Disastrously Low. And HRT Might Be Part of the Solution.

Sperm counts are disastrously low.

Like, WAY lower than you think.

In the United States, Europe, and Australia, the latest meta-analyses show a decline somewhere in the 50 to 60 percent range over the last 40 to 50 years.

Let that land for a moment.

Fifty years ago, men had twice as much sperm as they do today.

Meanwhile, endometriosis is up by 32 percent over the last seven years.

And PCOS?

From 1990 to 2021, global PCOS cases increased by 56 percent.

But here is the part that should make every provider pause.

The World Health Organization estimates that up to 70 percent of women with PCOS remain undiagnosed.

Seventy percent. Undiagnosed.

And if you have spent any time in clinical practice, you know this is true.

Men and women are suffering. Fertility is tanking. Hormonal instability is showing up earlier and earlier.

Pregnancies are more difficult. Cycles are more chaotic. Sperm quality is deteriorating. The younger generations are paying the price.

So the question becomes:

Why is this happening?

The list is long, but the top culprits are clear.

  • Endocrine-disrupting chemicals
  • Pesticides and herbicides
  • Environmental toxins in food, water, and air
  • Chronic stress
  • Ultra-processed diets
  • Sleep disruption
  • Metabolic decline

This is not stuff “out there.”

This is stuff that is literally in the water.

The authors of the major meta-analysis put it bluntly:

“Sperm count may sensitively reflect the impacts of the modern environment on male health throughout the life course.”
Levine et al., 2017

And I believe the same applies to female physiology.

Hormones do not operate in a vacuum.

When the environment collapses, so does the endocrine system.


We Cannot Fix the Entire World, But We Can Fix This Piece of It

I am not naive.

Fixing big agriculture, big pharma, and all the other “bigs” is not something you or I will accomplish by lunchtime.

But there is something we can do right now.

We can be David in a world full of Goliaths.

And the stone in our slingshot is hormone replacement therapy.

Because this crisis is not just about fertility.

It is about mitochondrial failure, metabolic breakdown, chronic inflammation, immune disruption, and endocrine collapse.

HRT is not “anti-aging.”

It is not “cosmetic.”

It is not “optional.”

HRT is primary care.

It is foundational.

And it is rapidly becoming one of the most important tools we have to support human physiology in a failing environment.

Not someday.

Right now.


HRT Is Not Just for the Elderly. It Never Was.

We have been taught to treat hormone replacement like something reserved for menopause or andropause.

But that mindset is not serving the world we live in today.

The modern environment is hitting men and women with hormone-disrupting forces at ages where this never used to happen.

Endometriosis, PCOS, low testosterone, infertility, anovulatory cycles, cortisol dysregulation, thyroid dysfunction, and metabolic collapse are happening younger. Much younger.

And HRT is one of the few tools that can step in and stabilize the physiological system before it completely derails.

That is why I keep saying this:

HRT is the future of medicine.

Not as a trend.

Not as a fad.

But as a necessary response to a world that is getting harder on the human endocrine system by the year.
